{"id":6061,"date":"2021-01-30T17:14:57","date_gmt":"2021-01-30T17:14:57","guid":{"rendered":"https:\/\/www.improvementscience.co.uk\/blog\/?p=6061"},"modified":"2021-05-04T05:54:44","modified_gmt":"2021-05-04T05:54:44","slug":"end-in-sight","status":"publish","type":"post","link":"https:\/\/hcse.blog\/?p=6061","title":{"rendered":"End In Sight"},"content":{"rendered":"<p>We are a month into Lock-down III.<\/p>\n<p><strong><em>Is there any light at the end of the tunnel?<\/em><\/strong><\/p>\n<p>Here is the reported UK data.\u00a0 As feared the Third Wave was worse than the First and the Second, and the cumulative mortality has exceeded 100,000 souls.\u00a0 But the precipitous fall in reported positive tests is encouraging and it looks like the mortality curve is also turning the corner.<\/p>\n<p>The worst is over.<\/p>\n<p><a href=\"https:\/\/www.improvementscience.co.uk\/blog\/wp-content\/uploads\/COVID_UKData_20210130.png\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-full wp-image-6062\" src=\"https:\/\/www.improvementscience.co.uk\/blog\/wp-content\/uploads\/COVID_UKData_20210130.png\" alt=\"\" width=\"883\" height=\"369\" \/><\/a><\/p>\n<p><em>So, was this turnaround caused by Lock-down III?<\/em><\/p>\n<p>It is not possible to say for sure from this data.\u00a0 We would need a No Lock-down randomised control group to keep the statistical purists happy and we could not do that.<\/p>\n<p><strong><em>Is there another way?<\/em><\/strong><\/p>\n<p>Yes, there is.\u00a0 It is called a <strong>digital twin<\/strong>.\u00a0 The basic idea is we design, build, verify and calibrate a digital simulation model of the system that we are interested and use that to explore cause-and-effect hypotheses.\u00a0 Here is an example: The solid orange line in the chart above (daily reported positive tests) is closely related to the dotted grey line in the chart below (predicted daily prevalence of infectious people).\u00a0 \u00a0Note the almost identical temporal pattern and be aware that in the first wave we only reported positive tests of patients admitted to hospital.<\/p>\n<p><a href=\"https:\/\/www.improvementscience.co.uk\/blog\/wp-content\/uploads\/COVID19_SEIRV_Jan2021.png\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-full wp-image-6063\" src=\"https:\/\/www.improvementscience.co.uk\/blog\/wp-content\/uploads\/COVID19_SEIRV_Jan2021.png\" alt=\"\" width=\"1000\" height=\"507\" \/><\/a><\/p>\n<p><strong><em>What does our digital twin say was the cause?<\/em><\/strong><\/p>\n<p>It says that the primary cause of the fall in daily prevalence of infectious people is because the number of susceptible people (the solid blue line) has fallen to a low enough level for the epidemic to fizzle out on its own.\u00a0 Without any more help from us.<\/p>\n<p>And it says that Lock-down III has contributed a bit by flattening and lowering the peak of infections, admissions and deaths.<\/p>\n<p>And it says that the vaccination programme has <strong>not<\/strong> contributed to the measured fall in prevalence.<\/p>\n<p><strong><em>What are the implications if our digital twin is speaking the truth?<\/em><\/strong><\/p>\n<p>Firstly, that the epidemic is already self-terminating.<br \/>\nSecondly, that the restrictions will not be needed after the end of February.<br \/>\nThirdly, that a mass vaccination programme is a belt-and-braces insurance policy.<\/p>\n<p>I would say that is all good news.\u00a0 The light the end would appear to be in sight.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>We are a month into Lock-down III.
