{"id":5788,"date":"2018-12-24T15:56:41","date_gmt":"2018-12-24T14:56:41","guid":{"rendered":"http:\/\/www.improvementscience.co.uk\/blog\/?p=5788"},"modified":"2024-09-07T07:40:34","modified_gmt":"2024-09-07T07:40:34","slug":"reflect-and-celebrate","status":"publish","type":"post","link":"https:\/\/hcse.blog\/?p=5788","title":{"rendered":"Reflect and Celebrate"},"content":{"rendered":"<p style=\"text-align: left;\">As we approach the end of 2018 it is a good time to look back and reflect on what has happened this year.<\/p>\n<p style=\"text-align: left;\">It has been my delight to have had the opportunity to work with front-line teams at University Hospital of North Midlands (UHNM) and to introduce them to the opportunity that health care systems engineering (<a href=\"http:\/\/www.healthcaresystemsengineering.uk\">HCSE<\/a>) offers.<\/p>\n<p style=\"text-align: left;\">This was all part of a coordinated, cooperative strategy commissioned by the Staffordshire Clinical Commissioning Groups, and one area we were asked to look at was unscheduled care.<\/p>\n<p style=\"text-align: left;\">It was not my brief to fix problems.\u00a0 I was commissioned to demonstrate how a health care systems engineer might approach them.\u00a0 The first step was to raise awareness, then develop some belief and then grow some embedded capability &#8211; in the health care system itself.<\/p>\n<p style=\"text-align: left;\">The rest was up to the teams who stepped up to the challenge.\u00a0 So what happened?<\/p>\n<p style=\"text-align: left;\">Winter is always a tough time for the NHS and especially for unscheduled care so let us have a look\u00a0 and compare UHNM with NHS England as a whole &#8211; using the 4 hour A&amp;E target yield &#8211; and over a longer time period of 7 years (so that we can see some annual cycles and longer term trends).<\/p>\n<p style=\"text-align: left;\"><a href=\"http:\/\/www.improvementscience.co.uk\/blog\/wp-content\/uploads\/2018\/12\/UHNM_vs_NHSE_AE_Monthly_Yield.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-full wp-image-5789\" src=\"http:\/\/www.improvementscience.co.uk\/blog\/wp-content\/uploads\/2018\/12\/UHNM_vs_NHSE_AE_Monthly_Yield.jpg\" alt=\"\" width=\"659\" height=\"484\" \/><\/a><\/p>\n<p style=\"text-align: left;\">The A&amp;E performance for the NHS in England as whole has been deteriorating at an accelerating pace over the 7 years.\u00a0 This is a system-wide effect and there are a multitude of plausible causes.<\/p>\n<p style=\"text-align: left;\">The current UHNM system came into being at the end of 2014 with the merger of the Stafford and Stoke Hospital Trusts &#8211; and although their combined A&amp;E performance dropped below average for England &#8211; the chart above shows that it did <strong>not<\/strong> continue to slide.<\/p>\n<p style=\"text-align: left;\">The NHS across the UK had a <strong>very bad time<\/strong> in the winter of 2017\/18 &#8211; with a double whammy of sequential waves of Flu B and Flu A not helping!\u00a0 UHNM dipped badly too.<\/p>\n<p style=\"text-align: left;\">But look at what happened at UHNM since Feb 2018.\u00a0 Something has changed for the better and this is a macro system effect.\u00a0 There has been a positive deviation from the expectation with about a 15% improvement in A&amp;E 4-hr yield.\u00a0 That is outstanding!<\/p>\n<p style=\"text-align: left;\">Now, I would say that news is worth celebrating and shouting &#8220;<em>Well done everyone<\/em>!&#8221; and then asking &#8220;<em>How was that achieved?<\/em>&#8221; and &#8220;<em>What can we all learn that we can take forward into 2019 and build on?<\/em>&#8220;<\/p>\n<p style=\"text-align: left;\">Merry Christmas.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>As we approach the end of 2018 it is a good time to look back and reflect on what has happened this year.
