{"id":5422,"date":"2017-04-09T08:06:52","date_gmt":"2017-04-09T07:06:52","guid":{"rendered":"http:\/\/www.improvementscience.net\/blog\/?p=5422"},"modified":"2017-04-09T08:06:52","modified_gmt":"2017-04-09T07:06:52","slug":"storyboard","status":"publish","type":"post","link":"https:\/\/hcse.blog\/?p=5422","title":{"rendered":"The Storyboard"},"content":{"rendered":"<p><a href=\"http:\/\/www.improvementscience.co.uk\/blog\/wp-content\/uploads\/2017\/04\/Storyboard.png\"><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-5423 aligncenter\" src=\"http:\/\/www.improvementscience.co.uk\/blog\/wp-content\/uploads\/2017\/04\/Storyboard.png\" alt=\"\" width=\"500\" height=\"100\" \/><\/a><\/p>\n<p style=\"text-align: left\">This week about thirty managers and clinicians in South Wales conducted two experiments to test the design of the <a href=\"https:\/\/www.improvementscience.co.uk\/workshops\" target=\"_blank\" rel=\"noopener\">Flow Design Practical Skills One Day Workshop<\/a>.<\/p>\n<p style=\"text-align: left\">Their collective challenge was to diagnose and treat a &#8220;chronically sick&#8221; clinic and the majority had no prior exposure to health care systems engineering (HCSE) theory, techniques, tools or training.<\/p>\n<p style=\"text-align: left\">Two of the group, Chris and Jat, had been delegates at a previous ODWS, and had then completed their Level-1 HCSE training and real-world projects.<\/p>\n<p style=\"text-align: left\">They had <strong>seen<\/strong> it and <strong>done<\/strong> it, so this experiment was to test if they could now\u00a0<strong>teach<\/strong> it.<\/p>\n<p style=\"text-align: left\">Could they replicate the &#8220;OMG effect&#8221; that they had experienced and that fired up their passion for learning and using the science of improvement?<\/p>\n<p style=\"text-align: left\"><!--more--><\/p>\n<hr \/>\n<p style=\"text-align: left\">The first phase is to replicate reality using a simulated one-stop clinic where the delegates play the role of the patients.<\/p>\n<p style=\"text-align: left\">Their first-hand experience was of queues, chaos, confusion, fire-fighting, frustration, anxiety and disappointment &#8211; i.e. &#8220;situation normal&#8221;.<\/p>\n<p style=\"text-align: left\">The first phase also generated a big set of raw event data, a meaningless table of numbers. \u00a0This actually held the clues to the diagnosis, and therefore to possible paths to a\u00a0<em>no-queues-or-chaos<\/em> design.<\/p>\n<p style=\"text-align: left\">That was the goal: calm efficiency; a high quality experience for all; a safe, productive and resilient service that everyone could be proud of.<\/p>\n<p style=\"text-align: left\"><!--more--><\/p>\n<hr \/>\n<p style=\"text-align: left\"><a href=\"http:\/\/www.improvementscience.co.uk\/blog\/wp-content\/uploads\/2017\/04\/Storyboard_Maps.png\"><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-5424 aligncenter\" src=\"http:\/\/www.improvementscience.co.uk\/blog\/wp-content\/uploads\/2017\/04\/Storyboard_Maps.png\" alt=\"\" width=\"500\" height=\"164\" \/><\/a><\/p>\n<p style=\"text-align: left\">In medicine we start the diagnostic process by taking a<em> history. <\/em>We first need to build a mental picture of the context of the patient&#8217;s presenting complaint(s) &#8211; their symptoms and signs.<\/p>\n<p style=\"text-align: left\">In HCSE, we call this the <strong>Map<\/strong> phase and the image above shows some of the maps that the facilitators-under-test drew from the delegates.<\/p>\n<p style=\"text-align: left\">The &#8220;symptoms&#8221; of the &#8220;poorly process&#8221; are captured on the third map, the 4N Chart\u00ae, and specifically as the list of Niggles.<\/p>\n<hr \/>\n<p style=\"text-align: left\">The next phase is called <strong>Measure<\/strong> and for this the delegates formed into teams and, assisted by the facilitators-in-training, worked together to bring the data to life. \u00a0To make the data visible and informative. \u00a0To help the system tell its own story.<\/p>\n<p style=\"text-align: left\">This is the management equivalent to the <em>examination<\/em> and <em>investigation<\/em> phase of the medical model that all clinicians are taught.<\/p>\n<hr \/>\n<p style=\"text-align: left\">The groups were now ready to start the critical phase &#8211; <strong>Diagnosis<\/strong> &#8211; and to facilitate that step, all the maps and measures were arranged on the wall in the form of a <em>storyboard<\/em>.<\/p>\n<p style=\"text-align: left\">And using the storyboard and another HCSE technique called a DLCA analysis, the diagnosis of the root cause of the queues and chaos became apparent. \u00a0And it was <strong>not<\/strong> what the delegates expected!<\/p>\n<p style=\"text-align: left\">With that new insight the facilitators-in-progress introduced the delegates to a simple, practical, visual yet powerful HCSE design technique &#8230; and the delegates cooked up their new designs in a matter of minutes &#8230; on their own.<\/p>\n<p style=\"text-align: left\">And using the same tool, they predicted the future performance of their new design.<\/p>\n<hr \/>\n<p style=\"text-align: left\">So, all that was left was to test their new design &#8230; which they did &#8230; and &#8230; the queues and chaos had evaporated. \u00a0Just as they had predicted.<\/p>\n<p style=\"text-align: left\"><strong>OMG!<\/strong><\/p>\n<p style=\"text-align: left\"><em>(NB.
