{"id":130,"date":"2010-01-17T17:27:18","date_gmt":"2010-01-17T17:27:18","guid":{"rendered":"http:\/\/www.saasoft.com\/blog\/?p=130"},"modified":"2010-01-17T17:27:18","modified_gmt":"2010-01-17T17:27:18","slug":"omission-and-comission","status":"publish","type":"post","link":"https:\/\/hcse.blog\/?p=130","title":{"rendered":"Errors of Omission and Commission"},"content":{"rendered":"<p>I like\u00a0doodling on Post-It\u00ae Notes\u00a0and playing with two-by-two tables and recently I came across one that triggered a bit of an &#8220;Eureka&#8221;<a href=\"http:\/\/www.improvementscience.co.uk\/blog\/wp-content\/uploads\/2010\/01\/Action_and_Outcome.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-thumbnail wp-image-134\" title=\"Action and Outcome\" src=\"http:\/\/www.improvementscience.co.uk\/blog\/wp-content\/uploads\/2010\/01\/Action_and_Outcome-150x150.jpg\" alt=\"\" width=\"150\" height=\"150\" srcset=\"https:\/\/hcse.blog\/wp-content\/uploads\/2010\/01\/Action_and_Outcome-150x150.jpg 150w, https:\/\/hcse.blog\/wp-content\/uploads\/2010\/01\/Action_and_Outcome-300x300.jpg 300w, https:\/\/hcse.blog\/wp-content\/uploads\/2010\/01\/Action_and_Outcome-100x100.jpg 100w, https:\/\/hcse.blog\/wp-content\/uploads\/2010\/01\/Action_and_Outcome.jpg 751w\" sizes=\"auto, (max-width: 150px) 100vw, 150px\" \/><\/a> moment.<\/p>\n<p>The two dimensions were Action (Nothing-to-Something) and Outcome (Worse-to-Better).\u00a0 We are all familiar with the good feeling that comes from doing something and seeing things get better; and the not-so-good feeling of doing something and seeing things\u00a0get worse!\u00a0 I discovered that this latter option is called the &#8220;Error of Commission&#8221; and is the one we fear most because we leave\u00a0an audit trail of evidence that can be traced\u00a0back to our action. It does not seem to matter that we did not\u00a0intend the outcome to be worse.<\/p>\n<p>However, the 2 x 2\u00a0table also suggests that there are two other combinations. How do we feel when we do nothing and things get better?\u00a0What do we learn from that experience? And how do we feel when we do nothing and things get worse?\u00a0This, I discovered,\u00a0is called the &#8220;Error of Omission&#8221;\u00a0and is an error that is\u00a0more difficult to learn from\u00a0because\u00a0there is no audit trail of cause-and-effect evidence. It is also the error that generates the greatest sadness &#8211; a feeling of loss of what might have been.<\/p>\n<p>Both the Error of Commission and the Error of Omission\u00a0can lead to unintended negative consequences.\u00a0 It appears that our systems\u00a0are better designed to manage the Errors of Commission.
